STOCKHOLM (Reuters Health) Jul 22-2002 - The investigational drug memantine slows cognitive decline in patients with advanced Alzheimer's disease compared with placebo, researchers reported here Sunday at the 8th International Conference on Alzheimer's Disease and Related Disorders. Memantine is a N-methyl-D-aspartate (NMDA) receptor agonist, which modulates glutamate levels. Glutamate is known to be neurotoxic; therefore memantine is hoped to be neuroprotective. Dr. Anders Wimo of the Karolinska Institute in Sweden and colleagues randomized 252 patients with advanced Alzheimer's to either memantine or placebo for 28 days. Dr. Wimo described the patients as requiring a high level of care (including personal care) throughout the day, and a high level of supervision. At the end of the study period, treated patients required significantly less caregiver time compared with placebo patients--more than 51 fewer hours per month. Five patients in the placebo group required institutionalization during the study period compared with one in the memantine group. The drug was well tolerated and showed no major adverse effects.
